Consent Form Language for Anonymous or Identified Web Surveys

This is an anonymous (or confidential, if it is not anonymous) survey. As an online participant in this research, there is always the risk of intrusion, however small, by outside agents (i.e., hacking) and, therefore the possibility of being identified exists. No absolute guarantees can be made regarding the confidentiality of electronic data. However, the data collected in this survey will be transmitted in encrypted format to provide additional safeguards against hacking. This helps ensure that any data intercepted during transmission cannot be decoded and that individual responses cannot be traced back to an individual respondent.
