Guidelines for writing your autobiography for the medical, dental and veterinary school application process
The purpose of the autobiography is for applicants to reflect upon the motivation for a career in the health professions and to share important information with the prehealth advisor about the academic, extracurricular, and service activities. Oftentimes the autobiography can serve as a springboard for writing the application essays. The autobiography is only for the prehealth advisor; it is not shared with anyone else on campus or sent to professional schools.
Recommendation request form
Applicants must give a signed copy of this form to all recommenders, and the recommender must return a signed copy of the form along with a signed recommendation letter on letterhead to the Office of Health Professions Advising. Letters of recommendation for medical and dental school are due by June 1 of the application year.
