Schools Lists:

Note:  If you choose to submit the schools list by email rather than sending this office a printed copy, please rename the schools list file with your last name, first initial and schools list - ie SmithJohnmedschlist.pdf or SmithJohnmedschlist.doc .  Do not include periods in the file name other than the period before the file extension; file names that include more than one period are rejected by virus scanning software.
