Flexible Benefit Plan (cont'd)

DENTAL PLAN

Delta Dental is a dental program that offers the flexibility of using any dentist, or using a Delta Dental of Pennsylvania participating dentist. A participating dentist has agreed to accept Delta Dental's usual, customary and reasonable (UCR) calculations as payment in full. Coverage is 100% of UCR for preventive, diagnostic and basic services. Coverage is 50% of UCR for major services, and for child orthodontics. There is an annual maximum of $1,000 per calendar year, and an $1500 lifetime maximum for orthodontics. The annual calendar year deductible of $50 per person ($150 per family) is waived for preventive and diagnostic services.

COBRA (Consolidated Omnibus Budget Reconciliation Act)

Continuation of dental coverage is available through Bryn Mawr College to all qualified individuals under the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA), as amended. Coverage is available for the statutory periods at the employee's expense.

ADDITIONAL TERM LIFE INSURANCE

You may purchase additional term life Insurance in increments of $10,000, to a maximum of $300,000. Employees under the age of 60 have a guaranteed issue amount of $50,000, provided that they enroll within 31 days of initial eligibility in the Flexible Benefit Plan. For employees ages 60-69, the guaranteed issue amount at the time of initial eligibility is $10,000. There is no guaranteed issue for employees who are age 70 or older at the time of initial eligibility.

Any amount of additional employee life insurance that is not guaranteed issue is subject to medical underwriting by the insurance carrier, The Hartford, at the time of the election. You will need to complete a medical questionnaire and submit it to the insurance carrier immediately upon election. The insurance carrier will advise whether your request for additional life insurance is approved or disapproved.

You may purchase dependent term life Insurance for your spouse or domestic partner in increments of $10,000, to a maximum of $300,000. Your spouse or domestic partner will also be subject to medical underwriting based on the same rules that apply to employees.

You may purchase $2,500; $5,000; $7,500; or $10,000 dependent term life Insurance for your children up to age 19 (to age 23 if full-time student) without satisfying evidence of insurability at the time of initial eligibility in the Flexible Benefit Plan. Subsequent enrollment is subject to medical underwriting.
