The Application can be filled out online and printed using any standard printer. In order to access the form, you must have the Adobe Acrobat plug-in, available free from http://www.adobe.com/products/acrobat/readstep2.html.

Please Note: Field contents entered using the free Acrobat Reader software cannot be saved. Selecting "save" from within the Acrobat Reader program, will only save a copy of the application with empty fields.

Text can be cut and pasted from word processing documents into the individual fields in the form. However, please note that when fields continue onto other pages, text must be cut and repasted to fit each individual field.

Acrobat will not print the contents of an "active" field on the form — i.e. one where the cursor is blinking next to the type. Click outside a field to "set" the type and make it printable.
